Appointment Scheduling

General Information

Scheduling Note: This request for an appointment does not guarantee an appointment will be able to be scheduled on the date or at the time that has been submitted using this form. A representative of the MPOSMI Administrative Staff will need to contact the requestor to confirm the appointment date and time to be scheduled.
 
Please arrive 10-15 minutes early for your Appointment for registration and to fill in a health questionnaire.

We will try to schedule an appointment at a time that is convenient for you. Because some visits require more time than others, please provide the receptionist with as many details as possible when making your appointment. If you are a new patient to our clinic or have not been seen for several years, you may download and print out the Patient History Form and the Patient Information Form prior to your appointment to save you time in registering.

When you come for your Appointments, please remember to bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
